| Forms |
| Forms are an integral part of sponsored programs. Some of these forms are used during and prior to a grant application, while others are used during the execution of a sponsored program to report back to the funding agency. |
| You will need Adobe Acrobat to access many of the available forms, though some sites have versions available in MS Word. There are several other formats being used, primarily by government sites. Some of these document types require specialized readers, (e.g., Accessible FormNet, Formflow) and store only a data file on your local hard drive. It is recommended that you use Adobe Acrobat, or MS Word forms when possible so that the files can be safely archived and you are not dependant upon the stability of an external web-site. |
| It is good practice to save both paper and digital copies of all forms you complete. |

| Form Name | Comments |
| Grant Proposal Routing Form | This form is required to obtain institutional approval for a grant/contract application prior to its submission. Before significant investments in time are made in developing a proposal, the concept, budget and personnel implications, should be discussed with the author's supervisor and other personnel who may have useful perspectives on the feasibility and implications of the proposed project. |
| Prior Approval Form | An internal form required to seek approval for specified types of changes to grants and/or grant budgets. Click HERE for the policy regarding the form's use. |
| Requisition Form | An Excel form to be used when submitting a purchase requisition. Directions for completing this form can be found HERE. The new form and procedure to be found here will go into effect November 8, 2004. Right click on the link at left, choose "Save Target As" and save the Excel form to an appropriate directory. Open the file and customize the "Choices" worksheet to only include the line items and budgets that you use and then choose "Save As," under the File menu, and select "Template" in the "Save as type" drop-down list. (Or, conversely, ask for help.) |
| Retroactive Cost Transfer | An Excel template used to certify reassigning an expense from one budget to another retroactively. Refer to the Retroactive Cost Transfer policy. |
| Monthly Report Form | A form for internal use required of every sponsored program director on a monthly basis. Please contact the Office of Sponsored Programs for additional information. Right click on the link at left, choose "Save Target As" and save the Word file to an appropriate directory. Open the file and then choose "Save As," under the File menu, and select "Document Template (*.dot)" in the "Save as type" drop-down list. (Or, conversely, ask for help.) |
| Disclosure Form | Draft Form and proposed policy!! A form required annually from all sponsored program administers (and some others) to declare any potential conflicts of interest. Refer to Conflict of Interest Policy. |
| The tables below have links to a variety of forms frequently used in grant applications and management with federally funded projects. These are the Standard Forms, (the ubiquitous SF ###) common to most federal programs and can be filled in on the computer using Adobe Acrobat; the Federal Government no longer generally makes these forms available in Word format. They can also be found and completed on your computer utilizing a program called "Accessible FormNet." The Standard Forms can be accessed in these 2 formats through this link, in addition to the links provided below. | ||

| FedForms | A government operated website. If you cannot find what you need through this site you are in trouble! Try the Agency Forms or the GSA Standard Forms links on this page. |
| Program Support Center | A great site managed by the Department of Health and Human Services. Contains links to other government sites and enables access to many forms, including the Standard Forms used by many federal agencies. Forms are in Adobe Acrobat format and can be filled in on your desktop. |
| TRAM | This is a site hosted by Arizona State University. Provides access to forms used by a wide range of federal agencies, departments and administrations. (Also available in Mac versions!) |
| Department of Education | A direct link for ED forms in Word and PDF formats. The "PDF Form Version" files are the easiest to use. |
| National Park Service | Many of the forms are for the use of Park Service employees, but additional agency forms and links are provided. |
