All employment applications must include the following:

Note: Letters of recommendation are to be from individuals having knowledge of the candidate's skills, abilities, judgment and professional performance as they relate to the job in question gathered from observation of or first hand knowledge of the candidate's work experience…in other words letters of recommendation are to come from individuals having knowledge of the candidate's experience and qualifications. Relatives and personal friends are precluded from providing letters of recommendation unless they are by coincidence in a position to provide the type of first hand knowledge being requested.
