Surveys: Examples of Problematic Questions and How to Fix Them

Steps to Successful Surveys

Remark OMR Forms in Microsoft Word

Will work nicely for paper surveys, test answer sheets, data sheets, etc.

For bubbles, use a capital "O" ARIAL 12pt Font, not Times-New Roman. If you want better bubbles, use the True-Type Bubble Font, downloaded and installed from the link below. The True-Type Bubble Font includes alphanumeric bubbles that work well for grids and repetitive rows of question responses.

The Principia Products web site has examples of survey and other data collection forms in Microsoft Word at                 http://www.principiaproducts.com/office/downloads.html

Some of the forms will need the following True-Type Fonts installed:

        Bubbles True-Type Font:

Principia Products has designed a true type font that will create encircled letters and numbers. The font was designed with Remark Office OMR in mind, so it will work well with your forms. When using the font, type your numbers and letters with spaces in between. 10 point size is ideal. To install the font, place the file omrbubbles.zip in your Windows\Fonts directory, unzip it, and then reboot your computer. This font may be used and distributed freely. Get the font from:                         http://www.principiaproducts.com/office/downloads.html

        3 of 9 True-Type Bar Code Font:

This is a public domain True Type font, not to be confused with the Borg 3 of 9 (Trekkies will understand this). The barcode patterns are duplicated across both upper and lower case alpha characters. On a laser or inkjet printer they print out nicely around 24pt size. Deviate too far from this and the bar space proportions suffer so that scanning difficulties may become a problem. Remember that absorbent paper used in ink jet printers can significantly reduce the space widths. To print a SPACE character, use an exclamation point (!). To use the Code 3 of 9 font, place the unzipped code39.zip font in your Windows\Fonts directory and then reboot your computer. Get the font from the URL hyperlinked above in the bubbles font section.

Important: The Code 3 of 9 barcode specification requires that you use an asterisk (*) as the first and last character. For example, if you wanted to use the following text "abcd1234" as a Code 3 of 9 barcode, you would actually type: "*abcd1234*". If the text was "abcd 1234", you would type "*abcd!1234*". (do not type the quotation marks).
